Severe septal hypertrophy: is it necessarily a contraindication for the transcatheter implantation of an Edwards-Sapien prosthesis?

چکیده
Rev Esp Cardiol. 2010;63(2):240-52 241 aneurysm.4,5 High resolution computerized tomography (CT) or resonance angiography permit evaluation of the trunk and its branches.5,6 Etiologies like syphilis, tuberculosis, and collagenopathies should also be considered. Generally, the distinctive pathology of aortic aneurysms leads to a different management approach.4 Even if it is unclear, surgery is indicated (prosthesis, reconstruction with pericardium, or arteroplasty) if the risk of rupture exists.7 Patients with <60 mm aneurysms, low pulmonary pressure, absence of congenital or acquired significant right shunt or not associated with collagenopathies are considered at low risk and conservative management is preferred, as in our patient.1-3 To conclude, pulmonary aneurysms are extremely rare; they are frequently found in chest x-rays by chance. High resolution CT and resonance angiography are fundamental to diagnosis. Surgery is indicated in patients at risk of dissection or rupture.

Failed valve-in-valve transcatheter mitral valve implantation.
A 73-year old man underwent trans-septal anterograde implantation of a 26-mm Sapien-XT valve (Edwards Lifesciences, CA, USA) to treat a degenerated 27-mm Carpentier-Edwards bioprosthesis. During the follow-up, the patient experienced severe haemolysis with moderate-to-severe inter-valvular leak, and fluoroscopy revealed possible incomplete apposition of the Sapien-XT (Fig. 1). Reoperation was p...

BACKGROUND A new generation of balloon-expandable valves (e.g. Edwards SAPIEN XT) enables the use of a decreased sheath size using the NovaFlex™ delivery system for transfemoral transcatheter aortic valve implantation (TAVI). However, there are few data analysing the efficacy and safety of this new prosthesis. AIMS To evaluate periprocedural and 30-day clinical outcomes using the Edwards SAPI...

Transcatheter aortic valve implantation (TAVI) is a new method for patients with severe aortic stenosis at high surgical risk, such as previous cardiac surgery. The presence of mechanical mitral prosthesis might complicate TAVI because of possible interference between the both prosthesis. Some reports have already demonstrated the feasibility of TAVI in such patients. We report our experience w...

Extensive mitral annulus calcifications are considered a contraindication for valve surgery. We describe the case of a 76-year-old female with severe mitral and aortic stenosis associated with extensive calcifications of the heart. The patient underwent an open mitroaortic valve replacement using transcatheter aortic valve implantation with an Edwards SAPIEN XT valve (Edwards Lifesciences Corp....

Surgical aortic valve replacement (AVR) is the conventional treatment for severe aortic stenosis (AS). However, a high operative mortality of 7% to 10% is well recognized in high-risk groups.1,2 Risk of AVR is increased by a number of factors, including increasing age, and comorbidities, such as heart failure, respiratory and renal disease, prior cardiac surgery, and need for concomitant corona...
